At Kroger Health, my work encompassed defining the profiles and account management for clinic patients and conceptualizing a unified health account management experience for clinic, pharmacy, and nutrition.

Problem: in the fracas of providing ad-hoc care, numerous duplicate patient accounts were created (and continued to be created), which increased operational costs stemming from:

  • manual patient records reconciliation
  • staff inefficiency
  • revenue loss from denied claims
  • higher malpractice liability from increased patient safety risks like medication errors and misdiagnoses.

Solution: establish a process for unifying patient records into an Enterprise Master Patient Index.

As a facilitator, I held workshops to bring key actors together to plan the integrations required to achieve the goals of patient identification and data consolidation that would lead to:

  • improved data quality
  • enhanced care coordination
  • reduced administrative costs
  • integration with grocery and nutrition.

With many teams working to create a complete patient/customer profile and a consolidated health and wellness account management experience, it was crucial to visualize what data points were needed and to highlight which teams were responsible for them.
